I provide consent for the Australian Sports Commission (ASC) to:

1. Include my first name and my image (if provided), my club, sport, volunteer role and volunteer story (my Volunteer Details) on the interactive digital National Volunteer Map which will be hosted on a public website.
2. Use my Volunteer Details for the promotion of National Volunteer Week and other ASC initiatives, which may include their incorporation in marketing communications, such as emails, newsletters, web content and social media posts.
3. Send me communications related to the National Volunteer Map and National Volunteer Week.

I confirm that I, and the volunteer I am nominating, are both 18 years of age or older.

I confirm that I have received consent directly from the volunteer I am nominating, to their being featured on the National Volunteer Map. They are aware that:

1. Their first name, image (if provided), club, sport, volunteer role, and volunteer story (their Volunteer Details) will be included on the interactive digital map, which will be hosted on a public website.
2. Their Volunteer Details may be used in National Volunteer Week promotions and other ASC initiatives. This may include their incorporation in marketing communications, such as emails, newsletters, web content and social media posts.
3. The ASC will send them communications related to my nomination, the National Volunteer Map and National Volunteer Week.

• I confirm that the information and content I have provided to the ASC is current and accurate to the best of my knowledge.
• Where an image is submitted I must either own the copyright in the image, or have the necessary permissions from the copyright owner to provide the image to the ASC for use as described in these Terms and Conditions.
• I will ensure that the information or content I provide is not threatening, obscene, defamatory, derogatory or otherwise illegal or unlawful.

1. The ASC will display the National Volunteer Map and conduct promotional activities, including those associated with National Volunteer Week, at its discretion.
2. The ASC does not guarantee that any information and content I submit will be used or published as part of the ASC’s activities.
3. The ASC may edit information and content I submit for the purpose of its activities, provided that it does so reasonably.

The personal information you provide will be collected, used and disclosed for the purpose of this initiative and will be handled by the ASC in accordance with the requirements of the Privacy Act 1988. The ASC Privacy Policy further outlines our personal information handling practices.
